Freshman/Sophomore/Junior

-Be actively engaged in Person-Centered Planning in Preparation for school IEP conferences

-Contact The Arc Network (www.arcind.org)  to gather as much information as possible about Medicaid Waiver and other services

-Apply for and gather identifications needed to apply for jobs; social security card, birth certificate, Indiana Picture Identification Card

-Learn more about guardianship

-Learn more about work supports through Indiana Vocational Rehabilitation and local providers that are available in your area for after you leave high school

-Contact the Arc Master Trust about estate planning options (www.arcind.org)

Senior Year

-Apply to Vocational Rehabilitation and learn about service providers in your area (Arc Rehab - Lebanon 765-482-6815) providing work supports

-Apply for priority Medicaid Waiver

-Learn about transportation options to work, etc through BATS (Boone Area Transit System (765-482-5220)

- Males register with the Selective Service (within 30 days of 18th birthday)

-Register to Vote (18th birthday)

-Apply for SSI (Supplemental Security Income) (18th birthday)
